Sleep with Me $3.99 In a moment of giggling chaos, Richard and Lelia sleep together while getting dressed for a holiday party. Arriving late and still flushed, they barely register a slight, drab woman who, like a shadow, threatens the happiness they’ve achieved. Their reality slowly shifts: Mysterious fragments of a story about a troubled child appear on RichardÂ’s e-mail. A friend confides his adulterous affair, and Richard voyeuristically hangs on every detail. Richard grows more ambivalent about the approaching birth of his child; he becomes distracted and remote, and Lelia suspects another woman. In her loneliness and longing Lelia makes a surprising decision that nearly destroys the life that she and Richard have worked so hard to create. Told with astonishing clarity from both Richard and LeliaÂ’s points of view, Sleep with Me is an extraordinary tale of obsession and desire that takes hold, body and soul, and doesn’t let go until the final page is turned. |

A Fly In Time $12.94 Dr. Dennis E. Donham was born in East St. Louis, Illinois. At the time Dr. Donham attended high school, East St. Louis was listed in Look magazine as an all-America city, a far cry from what it became. Dr. Donham became a career student affairs educator in academe, doing his utmost to encourage young adults to reach high in their personal expectations. In that career he had to meet numerous personal challenges to overcome his own humble background. His poems reflect that odyssey and demonstrate the joy and humor in a purposeful life. They include playful encounters with Paul McCartney and George S. McGovern, and serious musings of oriental philosophy gleaned from the author’s experiences in Nepal and India. Among the most moving are tributes to his parents, who never abandoned the bedside of a severely disabled daughter for fifty-two years; and a murdered cousin, who along with her husband made weekly food deliveries to the poor. His poems cover the full range of human emotion and are accessible to all readers. Each one, however complex or simple, relates a rich story. Dr. Dennis E. Donham received a bachelor’s degree in history from Knox College, and Ms.Ed. and Ph.D. degrees in higher education from Southern Illinois University. A victim of undiagnosed sleep apnea, Dr. Donham would often write poetry to lull himself to sleep. He never kept those poems, however. With treatment for his sleep apnea upon retirement, he discovered that he could often memorialize events with poems, as well as concisely describe thoughts or passing moments on walks. He discovered he soon had enough poems to publish his own volume. His poems reflect the musings of an examined life not available to many butwhose emotions have meaning to all. |

Acute and Emergent Events in Sleep Disorders $99.95 America is a 24/7 lifestyle. This makes sleep—especially disruptions in sleep—a pressing concern for many Americans. According to the National Sleep Foundation (NSF), approximately 40 million Americans suffer from chronic sleep disorders, and an estimated 20-30 million others experience sleep-related problems. Chronic sleep disorders may also lead to psychiatric disorders such as depression and schizophrenia. Moreover, neurological disorders such as seizures, strokes, Parkinson’s, etc, and medical disorders such as asthma or arrhythmia, also affect the quality of sleep Americans receive.Acute and Emergent Events in Sleep Disorders creates awareness for the management of disorders that occur during sleep. Chokroverty and Sahota bring greater awareness to the treatment of sleep disorders, as well as treatments of neurological, medical, and psychiatric disorders. The book has six different sections covers a wide range of topics dealing with how to treat and manage these events. For example, when to prescribe CPAP (Continuous Positive Airway Pressure) machines for sleep apnea patients’ whose risks are doubled for stroke or death, as compared to those without the disorder. Another segment discusses treatment of Restless Legs Syndrome (RLS), a movement disorder which is amplified when trying to rest. Managing depression, which affects patients’ sleep cycles, is analyzed as well as its relation to sleep-deprivation and insomnia. Considerations for sleep disorders in children, such as Sudden Infant Death Syndrome (SIDS) are also presented. This book serves an effective tool for neurologists, clinical neuroscientists, residents, and fellows. |

Beyond Alzheimer’s: How to Avoid the Modern Epidemic of Dementia $16.72 This groundbreaking explanation of the causes and treatment of Alzheimer’s disease rests on the author’s belief that Alzheimer’s is merely one of several types of dementia—and that in most cases dementia is avoidable. He further explains that the various forms of dementia may well be different manifestations of the same set of underlying problems. Rather than being the inevitable result of aging, the author asserts dementia is primarily the result of bad diet, stress, lack of mental and physical exercise, and other poor lifestyle choices. Dr. Mendelson begins his book with a straightforward explanation of how the brain ages—physically, structurally, and chemically. He then explains the various methods for diagnosing dementia, as well as how it can often be misdiagnosed if a person has suffered a head injury or stroke, has a hormone or vitamin deficiency, or is taking a medication whose side effects can mimic dementia. The remainder of the book is prescriptive, and offers hope to both Alzheimer’s patients and their caregivers by recommending scientifically tested herbs, vitamins and nutraceuticals that can help mitigate or delay the effects of dementia. Finally, the author suggests lifestyle changes that might help a person avoid dementia altogether, commonsense health tips that include steps to prevent heart disease and diabetes, treatment for sleep apnea, maintaining an ideal body weight, and even engaging in a more active social life. |

Cerebral blood flow regulation in intermittent hypoxia. $49.99 Obstructive sleep apnea (OSA) patients are at increased risk of stroke independent of other risk factors; however, this mechanism remains unclear. This increased risk may be related to the OSA patient’s impaired ability to regulate brain blood flow. The purpose of this study was to examine the effects of acute and chronic exposure to intermittent hypoxic apneas (IHA) on dynamic cerebral blood flow regulation in healthy subjects. Each subject performed one 30-s hypoxic apnea every one minute for 20 min on one day (acute protocol) or for 10 days (chronic protocol). During each hypoxic apnea event arterial oxygen saturation reached 80-85%. The rate of regulation (RoR) was calculated as an index of dynamic cerebral autoregulation (CA) from the response of arterial blood pressure and cerebral blood flow velocity to acute hypotension produced by the cuff release technique. The RoR during hypotension was significantly attenuated following the IHA protocol (0.78 +/- 0.09 to 0.47 +/- 0.07 sec; P = 0.003), indicating a loss of dynamic CA as a result of IHA. In addition, RoR was significantly attenuated on days 1 and 10 of the chronic protocol from pre- to post-IHA values (0.78 +/- 0.11 to 0.64 +/- 0.10 sec-1; P = 0.016 and 0.87 +/- 0.15 to 0.46 +/- 0.05 sec-1; P = 0.026). The fluctuations of mean arterial pressure (MAP) and middle cerebral artery blood velocity (MCAV) in response to a single hypoxic apnea were not different before and after intermittent apneas indicating that there was not a loss of cerebral blood flow regulation under a hypertensive stimulus. These findings indicate the increased risk of stroke in the early morning waking hours when the OSA patient faces an orthostatic challenge, such as rising from the supine to the upright position. |

Combating Sleep Disorders $35 Approximately 70 million people in the United States are affected by a sleep problem. About 40 million Americans suffer from chronic sleep disorders, and an additional 20-30 million are affected by intermittent sleep-related problems. However, an overwhelming majority of sleep disorders remain undiagnosed and untreated. Some of the most common ones are insomnia, restless legs syndrome, parasomnias, circadian rhythm disorders, excessive daytime sleepiness, night terrors, sleep apnea, periodic limb movement disorder, and narcolepsy. Long standing sleep disturbances can lead to nonproductive lifestyles, poor work performance, weight gain, difficulty with concentration, and social avoidance. Some disorders, like sleep apnea, can lead to even more serious problems such as high blood pressure, heart attack, and stroke. Therefore, there is an urgent need for this introduction to the subject of sleep disorders.Chapters include information on symptoms and problems that signal a possible sleep disorder, descriptions of sleep loss effects and their demographics, definitions of common sleep disorders, explanations of various medical and psychological tests for sleep disorder diagnosis, essential medical treatments, guided intervention to change thinking about sleep and related health habits, alternative interventions, follow-up and maintenance issues, and behavioral changes. Appendices include a list of accredited sleep laboratories and resources, suggested readings, and suggested exercises and activities. |

Eagles $0.01 Next to family and country, there is little else more important to the typical resident of the Delaware Valley than their Philadelphia Eagles. These rabid fans will brave an Arctic freeze, a tumultuous downpour, or a sweltering August night to watch their Eagles fly. The players have never been mere numbers to the faithful, but more like members of their extended families. Eagles: Where Have You Gone? takes an informative stroll down memory lane and includes many of the players who have made being an Eagles fan so much fun in Philadelphia. From Norm Van Brocklin to Jeff Kemp, Wilbert Montgomery to Cyril Pinder, Jim Skaggs to Guy Morris, Don Hultz to Tim Rossovich, the book looks not only at the playing careers of many former Birds, but also their lives after their playing days ended. Many former players have stayed in the game at some level and enjoyed coaching. Many fans, however, do not know which former Eagles player later became an NFL official, which former defensive standout became a salmon fisherman, which wide receiver became a respected judge, or which player now operates hospital-based centers treating sleep apnea. Trivia buffs will also be challenged to name the former Eagle who owns the NFL record for opponents’ fumble recoveries in one season with nine. Local icons such as Chuck Bednarik, Tom Brookshier, and Ron Jaworski have remained in the spotlight years after their playing days ended. But Eagles: Where Have You Gone? Tells fans about what happened to players like Pete Liske, Leroy Keyes, and Super Bill Bradley, all Philly favorites. It also answers questions about whether or not Tony Franklin found his other shoe or if Happy Feller is a happy fellow these days. The bookgoes beyond the helmet and shoulder pads and takes a look at the human side of these former guardians of the gridiron. While the litany of Philadelphia Eagles teams may not have been as successful as some would have liked over the years, many of the players have earned Medals of Honor |

Essentials of Geriatric Psychiatry $66.99 The Essentials of Geriatric Psychiatry presents the essence of The American Psychiatric Publishing Textbook of Geriatric Psychiatry, Third Edition, covering not only a wide range of important mental diseases of the elderly but also the so-called normal age changes that result in biological, social, and behavioral changes in older adults. Essentials of Geriatric Psychiatry reviews the knowledge that has transformed the practice of geriatric psychiatry. It is authored by both basic and clinical scholars—not only psychiatrists, but also contributors from relevant biomedical and behavioral disciplines—who make the sometimes complex material understandable. Its coverage maintains an eclectic orientation regarding theory and practice, beginning with demography and epidemiology of late-life disorders and physiological considerations of patient care, then continuing with all of the parent text’s chapters on diagnostic interviews—including chapters on laboratory tests and the neuropsychological assessment of dementia. Subsequent chapters address ten of the most prominent psychiatric disorders of the elderly and include recent insights into such matters as understanding the parameters of late-life depression and treating the seven recognized somatoform disorders. Each chapter ends with multiple-choice questions for self-assessment. Among the other topics considered are: • Differential diagnostic problems in distinguishing dementia from delirium or depression and in identifying specific dementia disorders• Insights on treating late-onset schizophrenia and identifying psychotic symptoms arising secondary to the dementia of Alzheimer’s disease• In-depth review of late-life bereavement, reflecting a wealth of recent research regarding intense and/or “complicated” grief• Sleep and circadian rhythm disturbances—including sleep apnea—and treatment of |

Essentials of Sleep Medicine: An Approach for Clinical Pulmonology $177.46 Sleep disorders represent a major portion of the chief complaints seen by pulmonologists and other physicians. Sleep apnea and hypopnea syndrome for example, are common disorders with significant adverse health consequences. Sleep apnea is associated with increased cardiovascular mortality, impaired quality of life and increased motor vehicle accidents. In addition, sleep apnea often co-exists with other chronic conditions including obesity, the metabolic syndrome, and tobacco use disorder. Patients with sleep-related conditions often present with non-specific complaints that require a broad and detailed knowledge of the wide range of sleep disorders. In Essentials of Sleep Medicine: An Approach for Clinical Pulmonology, a concise, evidence-based review of sleep medicine for the pulmonologist is presented. Providing a focused, scientific basis for the effects of sleep on human physiology, especially cardiac and respiratory physiology, chapters also outline a differential diagnosis for common sleep complaints and an evidence-based approach to diagnosis and management. This includes a review of the current standards of practice and of emerging technology and unresolved issues awaiting further research. Each chapter includes a summary of current research and outlines future research directions and issues. In all, Essentials of Sleep Medicine: An Approach for Clinical Pulmonology provides a clear diagnostic and management program for all the different sleep disorders, with a major focus on respiratory disorders of sleep, and includes key points and summaries. Developed by an international group of renowned authors, Essentials of Sleep Medicine: An Approach for Clinical Pulmonology is an invaluable resource for pulmonologists, respiratory care practitioners, polysomnographic technologists, graduate students, clinical researchers, and other health professionals seeking an in-depth review of sleep medicine. |

Excerpt: An alp is a nightmare creature originating in German folklore .Not to be mistaken with the similarly named Alp-luachra , the alp is sometimes likened to a vampire , but its behavior is more like that of the incubus . The word “alp” is also a variation on the word “elf “, and it also is known by the following names: trud , mare , mart , mahr , Schrat , and Walrider . Many other variations exist in surrounding European areas. The appearance of the alp itself is rarely described, other than it wears a hat and can shape-shift.An alp is typically male, while mara and mart appear to be more feminine versions of the same creature. Its victims are often female, and it usually attacks during the nighttime, controlling dreams and creating horrible nightmares. An alp attack is called an Alpdruck , or often “Alpdrücken”, which means “Elf Pressure”. An Alpdruck is when an alp sits astride a sleeper’s chest and becomes heavier until the crushing weight awakens the terrified (and breathless) dreamer. They awake terrified and unable to move under the alp’s weight. This may have been an early explanation for sleep apnea and sleep paralysis , as well as night terrors . It may also include lucid dreams . Alps may also enter the body by becoming a fine mist and entering through the nostrils and mouth, where it can |
